About Ninlib Nilotinib 150 mg Capsule

Medically reviewed by a registered oncology pharmacist. Content sourced from FDA prescribing information for Nilotinib, European Medicines Agency (EMA) assessment reports, NCCN CML treatment guidelines 2026, and peer-reviewed clinical trial data. Last updated: June 2026.

Ninlib 150mg is a generic Nilotinib capsule — the same active molecule as brand-name Tasigna (Novartis) — used to treat Philadelphia chromosome-positive chronic myeloid leukaemia (Ph+ CML) in adults. It is a second-generation BCR-ABL tyrosine kinase inhibitor (TKI) prescribed for newly diagnosed chronic phase CML and for patients who are resistant or intolerant to Imatinib (Gleevec). Manufactured by Hetero Drugs (WHO-GMP certified). Standard dose: 150mg or 300mg twice daily on an empty stomach. Priced at $643 per pack. Ships discreetly to USA, UK, UAE, and KSA with full customs documentation. Prescription required

Why Must Nilotinib Be Taken on an Empty Stomach?

Taking Nilotinib with food — especially a high-fat meal — significantly increases drug absorption. A high-fat meal can increase Nilotinib blood levels (AUC) by up to 82% compared to a fasted state. This elevated exposure substantially increases the risk of serious side effects including QT interval prolongation, which can cause life-threatening cardiac arrhythmias.  The strict rule: take Ninlib at least 2 hours before eating and wait at least 1 hour after taking your dose before eating. Swallow capsules whole with a full glass of water. Do not crush, chew, or open capsules.  Also avoid completely: grapefruit and grapefruit juice, Seville oranges, and starfruit — all of which inhibit CYP3A4 and can dramatically raise Nilotinib blood levels.

What Blood Tests and Monitoring Are Required With Nilotinib?

The following monitoring is mandatory during Nilotinib therapy — your oncologist will schedule these:  Before starting: ECG (baseline QTc interval), full blood count (FBC/CBC), liver function tests (LFTs), electrolytes (potassium and magnesium must be corrected before starting), lipid profile, fasting blood glucose.  During treatment: ECG repeated 7 days after initiation, then periodically. FBC and LFTs every 2 weeks for the first 2 months, then monthly. Blood glucose and lipid monitoring every 3 months. BCR-ABL transcript levels by PCR every 3 months to assess molecular response.

Is Ninlib as Effective as Tasigna?

Yes. Ninlib is a bioequivalent generic of Tasigna. Bioequivalence means that Ninlib delivers the same amount of Nilotinib into the bloodstream (within the standard 80–125% regulatory range) as the brand-name Tasigna when taken under the same conditions. The active molecule — Nilotinib — is chemically identical in both products. Hetero Drugs Ltd. manufactures Ninlib in a WHO-GMP certified facility, and the product meets international pharmaceutical quality standards.  For patients on long-term CML maintenance, switching from Tasigna to Ninlib (with oncologist approval) can reduce annual medication costs by 70–80% without any compromise in therapeutic outcome.

What Is the Difference Between Nilotinib 150mg and 200mg?

Ninlib is available in 150mg and 200mg capsule strengths. The strength used depends on your prescribed dose:  150mg capsules: Used for the standard 300mg twice-daily regimen in newly diagnosed Ph+ CML patients (2 × 150mg = 300mg per dose).  200mg capsules: Used for the 400mg twice-daily regimen in patients with resistant or intolerant CML (2 × 200mg = 400mg per dose).  Both strengths contain identical active ingredient. The capsule shell and excipients are the same. Do not substitute strengths without oncologist authorisation, as this changes total daily dose.

Can Nilotinib Cause Treatment-Free Remission (TFR)?

Yes — for a select group of patients who achieve a sustained deep molecular response (DMR), specifically MR4.5 (BCR-ABL transcript levels below 0.0032% IS) maintained for at least 2 years, stopping Nilotinib therapy under close medical supervision may be possible. This is called treatment-free remission (TFR) or 'functional cure' of CML.  The landmark ENESTfreedom trial demonstrated that approximately 48% of eligible patients who stopped Nilotinib maintained remission at 96 weeks. Patients must be closely monitored by PCR every 4–6 weeks for the first year after stopping, and every 3 months thereafter. If BCR-ABL levels rise above MR3 (major molecular response), Nilotinib should be restarted promptly — 97% of patients who restart achieve major molecular response again.  TFR is only appropriate for carefully selected patients under specialist oncologist supervision — it must never be self-initiated.

How it works

Nilotinib is a precision-targeted kinase inhibitor. In CML, a chromosomal translocation (the Philadelphia chromosome) creates a fusion gene called BCR-ABL, which produces an abnormal protein that drives uncontrolled proliferation of cancerous white blood cells. Nilotinib works by binding tightly to the BCR-ABL protein and blocking its kinase activity — switching off the molecular signal that tells leukaemia cells to multiply.  Compared to first-generation Imatinib, Nilotinib binds to the BCR-ABL kinase domain with approximately 30 times greater potency. This increased binding affinity is why Nilotinib is effective in patients whose CML has developed resistance to Imatinib. Nilotinib also inhibits c-KIT and PDGFR-alpha/beta kinases, though its primary therapeutic effect is through BCR-ABL inhibition.  The result: a significant, measurable reduction in BCR-ABL transcript levels (measured by PCR), leading to haematological remission, cytogenetic remission, and — for many patients — deep molecular remission (MR4 or MR4.5) with the possibility of treatment-free remission in long-term responders.
