What It Is

Linaclotide 72mcg Capsule contains Linaclotide — a guanylate cyclase-C (GC-C) agonist and the active molecule in branded Linzess® (AbbVie and Ironwood Pharmaceuticals, USA) and Constella® (EU). First FDA-approved in 2012, linaclotide has been prescribed to over 5 million patients across three indications. Fortune Business Insights

The 72 mcg dose specifically: This is the lowest available linaclotide dose — one of three available strengths (72 mcg, 145 mcg, 290 mcg). Understanding which dose is for which condition matters:

A lower dose of 72 mcg once daily is recommended for adults and children ages 6 to 17 with chronic constipation without a known cause, based on individual presentation or tolerability. If you have been prescribed 72 mcg, your gastroenterologist has selected the lowest effective dose — typically for patients who experienced diarrhoea on higher doses or those starting conservatively. Important November 2025 update: LINZESS received a label update in November 2025 expanding its IBS-C indication to include pediatric patients 7 years of age and older — making it the first FDA-approved treatment for IBS-C in children aged 7 and above. Vital Women Wellness

How It Works

Linaclotide works exclusively in the intestinal lining — it is not absorbed into the bloodstream in measurable amounts. This is its defining safety advantage over systemic constipation treatments.

When swallowed, linaclotide binds to guanylate cyclase-C (GC-C) receptors on the inner surface of intestinal epithelial cells. This binding activates the receptor, increasing intracellular cyclic GMP (cGMP). Elevated cGMP does two things simultaneously:

1. Increases fluid secretion into the intestine: Activates chloride and bicarbonate secretion into the gut lumen — softening stool and increasing intestinal fluid, accelerating transit.

2. Reduces abdominal pain: Elevated cGMP also activates sensory neurons that regulate visceral pain signalling — explaining why linaclotide reduces the abdominal pain and discomfort of CIC, not just constipation frequency.

Linaclotide is not expected to cause drug interactions because its metabolism occurs within the gastrointestinal tract and plasma concentrations are not measurable following administration of recommended dosage. This near-zero systemic absorption means essentially no drug interactions and no systemic side effects — a significant advantage over many constipation treatments. MDPI

What It Treats

Chronic Idiopathic Constipation (CIC): Linaclotide is indicated for CIC in adults. The AGA recommends using linaclotide for patients with IBS-C. CIC is constipation without an identifiable cause — fewer than 3 spontaneous bowel movements per week, hard/lumpy stools, straining, sensation of incomplete evacuation, and abdominal discomfort. The 72 mcg dose is the conservative starting point for adults who may be sensitive to diarrhoea. PubMed

Functional Constipation in children (6–17 years): FDA-approved for paediatric functional constipation in this age group. Dosing is weight-adjusted — prescribed by a paediatric gastroenterologist.

Not for IBS-C at 72 mcg: The IBS-C dose is 290 mcg orally once daily. If you have IBS-C, your doctor will prescribe the 290 mcg strength — not 72 mcg. PubMed

Dosage

Adults (CIC): 72 mcg orally once daily.

Take on an empty stomach, at least 30 minutes before the first meal of the day — this is the most critical administration instruction. Taking with food increases diarrhoea risk.

For patients who are unable to swallow the capsule whole, LINZESS capsules can be opened and administered orally with either applesauce or water. Do not crush or chew the capsule itself — open it and mix contents only. Cambridge Core

Missed dose: Skip the missed dose — do not double up. Take the next dose at the regular time the following day.

No dose adjustment required for renal or hepatic impairment — linaclotide is not renally or hepatically cleared.

Side Effects

Very common: Diarrhoea — the most common and dose-limiting side effect. Occurs in approximately 16% of patients at 145 mcg; the 72 mcg dose was specifically introduced to reduce this. If severe diarrhoea occurs, suspend dosing, rehydrate, and contact your doctor before resuming.

Common: Abdominal pain, bloating, flatulence.

Serious: LINZESS is contraindicated in patients less than 2 years of age due to the risk of serious dehydration. In older children and adults, severe diarrhoea causing significant dehydration requires immediate medical attention.

No systemic side effects — linaclotide is not absorbed into the bloodstream in meaningful amounts, so headache, dizziness, systemic allergic reactions, and drug interactions are not expected.

Who Should Not Use It

Contraindicated in: Children under 2 years — serious dehydration risk. Patients with known or suspected mechanical gastrointestinal obstruction — increased pressure risk.

Use with caution in: Patients with inflammatory bowel disease (Crohn's, ulcerative colitis) — not specifically studied. Patients with active diarrhoea — will worsen. Elderly patients — monitor hydration.

Not for: Diarrhoea-predominant IBS (IBS-D). Bowel obstruction. Acute abdominal conditions.

FAQ

What is the difference between linaclotide 72 mcg, 145 mcg and 290 mcg? The 72 mcg and 145 mcg doses are for chronic idiopathic constipation (CIC) — with 72 mcg being the lower-tolerability option. The 290 mcg dose is for IBS with constipation (IBS-C). Always use the dose your gastroenterologist has specifically prescribed for your condition.

Why take linaclotide on an empty stomach? Food significantly increases the rate of diarrhoea. Taking on an empty stomach at least 30 minutes before eating maintains effectiveness while minimising diarrhoea risk.

Is generic linaclotide available? Yes — Indian generics are available including Linzee 72mcg (Sun Pharma) and others at significantly lower cost than branded Linzess. These contain identical active substance.

Does linaclotide enter the bloodstream? No. Linaclotide acts entirely within the intestinal lining and is not detectable in plasma at recommended doses. This means no drug interactions and no systemic side effects.

Do you ship linaclotide 72 mcg internationally? Yes — to USA, UK, UAE, Canada, Russia, Japan, Thailand, Philippines, Malaysia, Australia.
